PADI Medical Statement

Before you arrive in Manado, you must be sure that you are in a suitable physical condition for the sport of diving. At the start of the course, every student must complete and sign a PADI Medical Statement, which includes a number of questions relating to your medical history. By answering the questions, you will discover if you'll require a doctor's check-up before taking to the water. (PADI insists on this if your medical history shows a 'contra-indication' to participating in diving.)

Please now follow these important instructions on how to check your medical history:
  1. Click this link: PADI Medical Statement (PDF file, 74kb)
  2. Print a copy
  3. Answer each question carefully. Sign and date the bottom of your statement
  4. If any of the answers you give to the questions are "yes", visit your doctor for a medical examination, taking your statement with you to show him/her*.
  5. If your doctor is satisfied with your health, you are ready to start your course. However, you must bring BOTH your signed Medical Statement and your doctor's clearance with you to Manado, as we are required to keep these with your training records.
If you are unsure of the answer to any of the questions, you should answer "yes".

Thank you for your cooperation!

We look forward to welcoming you to some superb diving here in Manado.
